Food Vendor Application

Please complete all required fields and upload all necessary documents.

Your form is considered incomplete if all areas are not filled out. Should you need to change anything on your form after submission, the form is not editable and you will need to resubmit another submission, in its entirety. So please make sure you have all your paperwork and information ready once you begin. You will need your Food License, a copy of your menu, photo of booth setup, a copy of your insurance listing Benton County Fair as an additional insured & your completed ST19.

To ensure a diverse selection of food for our fairgoers, we allow only one vendor to sell each specific food item. Only menu items listed and approved may be sold or advertised within your booth.

Vendors are not permitted to sell ice. Ice will be available for purchase at the fair office during office hours.

Due to the nature of the need to upload documents, this form may not be mobile-friendly and might require the use of a computer and/or tablet for proper submission of registration. You are also unable to save your information and come back later, so please make sure you have all proper documents needed before starting.

Submitting this form does not guarantee approval.

Upon approval, a team member will reach out with confirmation and include your booth payment invoice.
